Job Details Home Care Physical Therapist. 3 roles available. 1 - Queens Adult, 2 - Queens Pediatric. 3 - Nassau Pediatric
What you will be doing:
- Assessment and Evaluation:
- Home care physical therapists (PTs) assess the physical health and mobility of individuals within their home environment.
They evaluate the specific challenges and limitations faced by the patient, considering factors such as home layout, accessibility, and safety concerns.
Customized Treatment Plans:
- Based on the assessment, home care PTs develop personalized treatment plans to address the patient's unique needs and goals.
These plans often include a combination of exercises, therapeutic activities, and mobility exercises designed to improve strength, flexibility, and overall functional independence.
Education and Support:
- Home care PTs educate patients and their caregivers on proper techniques for exercises and activities.
They provide guidance on managing chronic conditions, preventing injuries, and promoting a safe home environment.
Continuous support and monitoring are crucial components of their role to ensure the ongoing effectiveness of the treatment plan and make necessary adjustments as needed.
Skills you need:
